When a cat starts going outside the litter box, it is never out of spite. Cats are telling you something: they are in pain, the box does not suit them, or something in their environment is stressing them.

Working through the possible causes methodically usually solves the problem.

Rule out medical issues first

Urinary tract problems, bladder inflammation, kidney disease, diabetes, constipation, and arthritis can all cause litter problems. A male cat straining to urinate with little or nothing coming out is an emergency — call a vet immediately, as a urinary blockage can be life-threatening.

Check the box set-up

Small details matter to cats:

Scoop at least daily and fully change litter regularly
Provide one box per cat plus one extra, in different locations
Use a large box — many commercial boxes are too small
Most cats prefer unscented, fine-grained litter
Keep boxes away from food bowls and noisy appliances
Older cats may need a low entry

Look for stress

New pets, visitors, moving furniture, outdoor cats visible through windows, or tension between household cats can all trigger litter problems. Give each cat safe resources and space, and reduce sources of conflict.

Clean accidents properly

Use an enzymatic cleaner designed for pet urine. Ordinary cleaners — especially ammonia-based ones — can leave a smell that attracts cats back to the same spot.
